Iz Torina stiže lijepa vijest o revolucionarnom zahvatu: liječnici u bolnici Molinette u Citta della Salute napravili su novu operaciju i omogućili potpuno slijepom pacijentu da ponovno vidi. Točnije, autotransplantacijom rožnice, bjeloočnice i spojnice od dva su slijepa oka rekonstruirali jedno koje vidi. 

Riječ je o izvanrednoj operaciji koju su prvi put u svijetu izveli profesor Michele Reibaldi (ravnatelj očne klinike Sveučilišta Molinette) i profesor Vincenzo Sarnicola, jedan od vodećih stručnjaka u svijetu kirurgije rožnice, piše Zimo.

"Kada sam se probudio i vidio obrise svojih prstiju i ruke, bilo je kao da sam se ponovno rodio", rekao je 83-godišnjak koji boluje od dviju ozbiljnih i različitih bolesti oka, zbog kojih je prije šest godina u potpunosti ostao bez vida. Zahvaljujući revolucionarnoj operaciji liječnici su mu uspjeli vratiti vid u desno oko i omogućiti mu prepoznavanje predmeta, lica i samostalno kretanje.

Kako su objasnili iz bolnice, pacijent je izgubio vid na lijevom oku prije 30 godina zbog ireverzibilne retinalne sljepoće, a u posljednjih 10 godina progresivno je gubio vid na desnom oku zbog rijetke kronične bolesti (očni pseudopemfigoid), koja mu je uništila rožnicu, a nažalost i površinu oka. U posljednjih nekoliko godina prošao je dvije tradicionalne transplantacije rožnice pune debljine na desnom oku, no obje su brzo propale zbog zatajenja površine oka.

Objašnjavaju kako je transplantacija rožnice pune debljine kirurški zahvat u kojem se samo rožnica koja je izgubila prozirnost zamjenjuje zdravom rožnicom preminulog darivatelja, piše Zimo.

"Normalno rožnica ima mnogo nižu stopu odbacivanja od drugih vaskulariziranih organa, ali u prisutnosti raširene promjene cijele površine oka, kao u slučaju pacijenta, taj rizik postaje vrlo visok", objasnio je prof. Sarnicola. "Konkretno, oštećenje matičnih stanica limbusa, područja između rožnice i konjunktive, rezultira nepovratnim neuspjehom transplantacije."

Ovom posljednjom operacijom prvi je put napravljena autotransplantacija cijele očne površine lijevog oka, koja nije uključivala samo rožnicu, već i dio bjeloočnice te cijelu spojnicu, uključujući i matične stanice limbusa.

"U ekstremnoj sintezi, zbog problema s mrežnicom, pacijent je nepopravljivo izgubio funkcionalnost svog lijevog oka, dok je desno oko zadržalo potencijal oporavka, koji se, međutim, pokazao uzaludnim kod tradicionalnih transplantacija", istaknuo je Reibaldi. 

"Operacija je izvedena tako da je iz lijevog oka, funkcionalno nepopravljivog, ali s rožnicom i očnom površinom u dobrom stanju, uzeta sva spojnica, sva rožnica i dva milimetra bjeloočnice, u jednom komadu, objasnili su liječnici. Dakle, trećina lijevog oka autotransplantirana je u desno oko, koje je potom obnovljeno i vraćen mu je vid.

Prava novost sastoji se u povećanju transplantata rožnice na cijelu površinu oka, na konjunktivno-skleralna tkiva, koja igraju temeljnu ulogu u omogućavanju uspjeha transplantacije u određenim uvjetima. Istodobno je lijevo oko rekonstruirano donorskim tkivom samo u estetske svrhe", objasnio je Sarnicola, piše Zimo.

Operacija je bila izvanredna i pacijent je nakon dva tjedna progledao i samostalno se kreće.

"Jako smo uzbuđeni i očekujemo trajni uspjeh na desnom oku jer je ono rekonstruirano vlastitim tkivima pacijenta i stoga potencijalno sigurno od problema odbacivanja koji su pogađali prethodne transplantacije", zaključuju Reibaldi i Sarnicola.

Iz Torina stiže lijepa vijest o revolucionarnom zahvatu: liječnici u bolnici Molinette u Citta della Salute napravili su novu operaciju i omogućili potpuno slijepom pacijentu da ponovno vidi. Točnije, autotransplantacijom rožnice, bjeloočnice i spojnice od dva su slijepa oka rekonstruirali jedno koje vidi. 

Riječ je o izvanrednoj operaciji koju su prvi put u svijetu izveli profesor Michele Reibaldi (ravnatelj očne klinike Sveučilišta Molinette) i profesor Vincenzo Sarnicola, jedan od vodećih stručnjaka u svijetu kirurgije rožnice, piše Zimo.

"Kada sam se probudio i vidio obrise svojih prstiju i ruke, bilo je kao da sam se ponovno rodio", rekao je 83-godišnjak koji boluje od dviju ozbiljnih i različitih bolesti oka, zbog kojih je prije šest godina u potpunosti ostao bez vida. Zahvaljujući revolucionarnoj operaciji liječnici su mu uspjeli vratiti vid u desno oko i omogućiti mu prepoznavanje predmeta, lica i samostalno kretanje.

Kako su objasnili iz bolnice, pacijent je izgubio vid na lijevom oku prije 30 godina zbog ireverzibilne retinalne sljepoće, a u posljednjih 10 godina progresivno je gubio vid na desnom oku zbog rijetke kronične bolesti (očni pseudopemfigoid), koja mu je uništila rožnicu, a nažalost i površinu oka. U posljednjih nekoliko godina prošao je dvije tradicionalne transplantacije rožnice pune debljine na desnom oku, no obje su brzo propale zbog zatajenja površine oka.

Objašnjavaju kako je transplantacija rožnice pune debljine kirurški zahvat u kojem se samo rožnica koja je izgubila prozirnost zamjenjuje zdravom rožnicom preminulog darivatelja, piše Zimo.

"Normalno rožnica ima mnogo nižu stopu odbacivanja od drugih vaskulariziranih organa, ali u prisutnosti raširene promjene cijele površine oka, kao u slučaju pacijenta, taj rizik postaje vrlo visok", objasnio je prof. Sarnicola. "Konkretno, oštećenje matičnih stanica limbusa, područja između rožnice i konjunktive, rezultira nepovratnim neuspjehom transplantacije."

Ovom posljednjom operacijom prvi je put napravljena autotransplantacija cijele očne površine lijevog oka, koja nije uključivala samo rožnicu, već i dio bjeloočnice te cijelu spojnicu, uključujući i matične stanice limbusa.

"U ekstremnoj sintezi, zbog problema s mrežnicom, pacijent je nepopravljivo izgubio funkcionalnost svog lijevog oka, dok je desno oko zadržalo potencijal oporavka, koji se, međutim, pokazao uzaludnim kod tradicionalnih transplantacija", istaknuo je Reibaldi. 

"Operacija je izvedena tako da je iz lijevog oka, funkcionalno nepopravljivog, ali s rožnicom i očnom površinom u dobrom stanju, uzeta sva spojnica, sva rožnica i dva milimetra bjeloočnice, u jednom komadu, objasnili su liječnici. Dakle, trećina lijevog oka autotransplantirana je u desno oko, koje je potom obnovljeno i vraćen mu je vid.

Prava novost sastoji se u povećanju transplantata rožnice na cijelu površinu oka, na konjunktivno-skleralna tkiva, koja igraju temeljnu ulogu u omogućavanju uspjeha transplantacije u određenim uvjetima. Istodobno je lijevo oko rekonstruirano donorskim tkivom samo u estetske svrhe", objasnio je Sarnicola, piše Zimo.

Operacija je bila izvanredna i pacijent je nakon dva tjedna progledao i samostalno se kreće.

"Jako smo uzbuđeni i očekujemo trajni uspjeh na desnom oku jer je ono rekonstruirano vlastitim tkivima pacijenta i stoga potencijalno sigurno od problema odbacivanja koji su pogađali prethodne transplantacije", zaključuju Reibaldi i Sarnicola.
